Login into your UKG account and submit a ticket. See how to submit a ticket on UKG here.
Sign into UKG. Select Pay under the Myself drop down tab. Select Income Tax. On the right-hand side under “Things I Can Do” select Add/Change Withholding Form (W-4).
Sign into UKG. Select Pay under the Myself drop down tab. Select W2 Tab. Select the applicable W2 year to download a copy. Once you select, you will be able to print in the upper right-hand corner.
Sign into UKG. Select Benefits under the Myself drop down tab. Select 1095-C Tab. Select the applicable year to download a copy. Once you select, you will be able to print in the upper right-hand corner.

If you are unsuccessful at recovering your password using the Forgot Password feature, then you will be locked out and must email the HR department to request a password reset.
Employees who are injured at work should report the injury to their supervisor immediately and complete a First Report of Injury Form to be submitted to the company’s workers compensation insurance carrier. If you will require time away from work due to the injury then you should also complete the Leave of Absence Request Form using this link: https://enhancetherapies.com/leave-of-absence-request/ and indicate that the leave is due to a work-related injury.
Sign into UKG. Select HR help under the Myself drop down tab. Select the type of question/request from the list of Categories on the right side of the page to submit your question. There are also informational articles and links to employee handbooks available from here.
Policy questions that may be found here include Paid Time Off (PTO), bereavement leave, jury duty, holidays, continuing education, etc.
